- Review the Vendor Health and Safety Requirements carefully.
- Complete and return the Vendor Acknowledgement Form by email to Procurement at [email protected].
- After the Vendor Acknowledgement Form is received, your University of Ottawa contact will follow up with you to complete the Vendor Contract Specific Health & Safety Information Form, which must also be submitted before starting any work on University property.
